In order to become qualified for breathing retraining classes, please provide (send) these details about yourself:
- Full name, year of birth or age
- City/region and country
- Height and body weight
- Current health problems and their duration
- Main symptoms and complaints now (write on extra space if needed)
- Medication and drugs you use now (with their dose and duration)
- Past health problems and their duration
- Your abilities to practice breathing exercises and do physical exercise every day.

Keep this info for future references since you will be asked to fill your Personal Questionnaire with many more questions related to your health state and current lifestyle. The initial information is required to evaluate if you can restore your health or not. It may be too difficult to recover health of people with combined severe health problems (e.g., metastatic cancer combined with complications of diabetes and numerous allergies). There is no guarantee that breathing of such patients can be normalized.

However, over 99% of people can expect their health restoration provided that a person will diligently practice breathing exercises (up to 1-2 hours per day) with gradually increasing intensity and amount of physical exercise (up to 2-3 hours per day) for long-term maintenance of health and vitality. Breathing retraining also involves lifestyle changes in sleep and diet of the students.
